 AGE IDENTIFICATION REQUIREMENTS:

Must be 16 years old or older. if under 18, a parent/legal guardian must be present during appointment and both must provide proper identification,  

 **NO EXCEPTIONS**

 

Parent/legal Guardian must show a valid State ID or DL. 

*In the situation of a legal guardian, must show proof of guardianship.

Minor must provide State ID or birth certificate and school photo ID.

 

TRAVEL FEE AND POLICY:

Travel Fee: A starting fee of $15 applies to all mobile services within a 30-mile radius.

Comfort Policy: I reserve the right to cancel the appointment if I feel uncomfortable at any point.

Client Responsibilities:

1. Pet Policy: Please ensure that all pets are put away during the appointment.

2. Setup Area: The room or area where the service will be provided must be prepared for equipment setup.

3. Safety Requirement: No additional people should be in the room during the appointment, as I operate a blue light, and protective glasses are only available for myself and the client.

DEPOSITS:

A non-refundable deposit is required to hold your appointment. The deposit amount will be applied towards the cost of your appointment. Upon booking, you'll receive an email with all aftercare instructions & FAQS.

PLEASE READ THIS EMAIL THROUGHLY.

If you need to reschedule, your deposit will be transferred to your new appointment if rescheduled at least 24-hours in advance of your scheduled appointment.

If cancellation is necessary, your deposit will be created to your account for future use, provided it is cancelled within 24 hours prior to your appointment.

In the event of a no call/no show, the deposit will be forfeited.

GUARANTEE:

30-Day Guarantee (Crystals & Opals): If your gem falls off within 30 days of the application, I will replace the gem at no cost.

90-Day Warranty(Crystals & Opals): If the gem falls off within the first 3 months, I will replace the gem for a discounted rate of $20. If a 18k Gold Charm falls off within 90 days of the application, I will replace the charm for a small $60 fee (wholesale cost of gem)

 

90-Day Warranty - 18k Gold Charms (including bezels): If still in your possession, free replacement up to 3 months after your appointment.

*** It is typical for a tooth gem to have a lifespan of 6-12 months, sometimes longer. The longevity of your gem is influenced by aftercare and how much wear and tear there is in thre area. ***
